Propionibacterium acnes endogen endoftalmit med bilateral
This patient reported a 3 month history of red sore photophobic eye due to chronic uveitis following routine cataract surgery performed elsewhere in 2003. Ex 2020-12-23 Cutibacterium acnes (formerly Propionibacterium acnes) is the relatively slow-growing, typically aerotolerant anaerobic, Gram-positive bacterium (rod) linked to the skin condition of acne; it can also cause chronic blepharitis and endophthalmitis, the latter particularly following intraocular surgery. Its genome has been sequenced and a study has shown several genes can generate enzymes for AUGUST 2007 I CATARACT & REFRACTIVE SURGERY TODAY I 29 RICHARD E . BR AUNSTEIN , MD This patient has a presentation consistent with postoperative P. acnes endophthalmitis. This case describes the utility of the histopathological technique of microdissection and polymerase chain reaction (PCR) for the diagnosis of delayed postoperative P. acnes is generally associated with delayed endophthalmitis that typically presents several months to two years postoperatively. P. acnes was cultured from the extracted implant. The endophthalmitis resolved completely after implant removal. CONCLUSIONS: Despite introduction of antibiotics into the intravitreal, intracameral, and subconjunctival space surrounding the Baerveldt implant, infection persisted until complete explantation of the device. P. acnes was cultured from the extracted implant. The endophthalmitis resolved completely after implant removal.

C. acnes is an uncommon cause of endophthalmitis after cataract surgery, typically presenting months after surgery as an indolent, smoldering inflammation [ 2, 3, 4 ].

Also some bacteria (P. acnes) may cause chronic inflammation with mild symptoms. P. acnes is part of typical skin flora and is usually inoculated against at the time of intraocular surgery. 20 This is a report of P acnes endophthalmitis presenting with discrete preretinal lesions where surgical and medical management lead to a complete resolution of uveitis and symptoms after a 3-year follow up where the patient’s final visual acuity was Snellen 20/20 OU.
